Bring this form along with your dental insurance card (if applicable) and a valid form of identification which you will present to our front office staff.

All patients under the age of 18 must be accompanied by a parent or guardian.
Day of your visit:
Quality dental care requires a team effort. We understand your first visit to a new dentist can be stressful. Your initial visit is the foundation to a long-term and trusting relationship with your dentist. Upon arrival to our office, you will be greeted and registered by one of our friendly staff. Then, you will be escorted to one of our private operatories. You will have the chance prior to your exam to discuss your dental health concerns and expectations. Then, we will perform advanced digital x-rays which instantaneously produce high resolution x-ray images of your teeth and jaw. Digital radiography produces 90% less radiation than traditional radiography systems. Dr. Ayoubi will perform a comprehensive dental examination. This involves a screening for oral cancer, a tooth-by-tooth charting to identify and record the current condition of your teeth, and a periodontal exam to assess the condition of your gums. State-of-the art intra-oral digital photographs will then be taken. Any abnormal findings from the digital x-rays and photographs will be presented to you on a high resolution monitor. You will see exactly the problems spots and receive a careful explanation of any dental work that would be needed. Dr. Ayoubi will provide you with an in-depth dental health report. She will discuss any necessary procedures that will be required to address your dental health needs as well as any alternatives or options. If any treatment will be needed, you will be presented with a comprehensive treatment plan and estimate. Often, your first visit will include a comprehensive general cleaning, though if it’s been a while since you last had your teeth cleaned, you may need to be scheduled for a longer visit at a later time.
We accept most major insurance plans and will gladly process your claim. Please have your insurance card and driver’s license with you at your first appointment. Insurance policies generally cover only a portion of the total treatment cost. Please remember that your dental insurance policy is an agreement between you and your insurance company, not between your insurance company and our office. We make no guarantee of any estimated coverage, but we will do our best to ensure that you receive your maximum benefits. Please keep in mind that you are responsible for your total obligation of fees for treatment provided should your insurance benefits result in less coverage than anticipated. If you would like to know what your expected coverage will be, we can submit a pre-authorization estimate to your insurance company. Your insurer will usually respond and send a detailed explanation of benefits within 4-6 weeks.
Unless other arrangements have been made, we ask that you pay your portion of the bill at the time of treatment. It is your responsibility to pay any balance not paid by your insurance company.

Appointments and Cancellations:
When we make your appointment, we are reserving a time slot and dental exam room for your particular needs. We ask that if you must change an appointment, please give us at least 24 hours advance notice. This courtesy makes it possible to give your reserved room to another patient who needs to be seen.
Missed appointments and cancelled appointments without 24 hours notice are subject to a cancellation fee of $40. Repeat cancellations or missed appointments will result in loss of future appointment privileges.
We feel that our patient’s time is valuable. When your appointment is made, a room is reserved, your records are prepared, and special instruments are readied for your visit. Except for emergency treatment for another patient, you can expect us to be prompt. We, of course, would appreciate the same courtesy from you.
Infection controls and universal precautions protect clients and staff alike. Everyone benefits from rigorous infection control — you, your dentist, and the dental team. The cornerstone of a good and safe dental practice is the element of trust. Allure Dental maintains the highest level of infection control to safeguard the health of our patients.
Dr. Ayoubi and our entire team follow procedures recommended by several federal agencies: the Occupational Safety & Health Administration (OSHA), the Environmental Protection Agency (EPA) and the Centers for Disease Control (CDC). These measures include:
- Disinfectant hand soap
- Gloves and face masks
- Chemical disinfection of countertops and surfaces
- Sterilization of all equipment before every use
- Disposable materials
We sterilize all reusable equipment, including dental hand pieces. We use an autoclave, a device that kills bacteria and viruses by steam, heat and pressure.
The best defense against disease is information. The more you know, the better equipped you are to make wise decisions about your health care. The more you know about our daily procedures and policies, the more comfortable you will feel.
